WHO HPV LabNet Newsletter No.5 Summary

1. WHO HPV Laboratory Manual

• A global reference manual by WHO HPV LabNet in final stages (led by GRL/CDC, US), expected by 2010.
• Purpose: To improve lab support for HPV vaccination programs by providing:
  • Brief on HPV biology, disease burden, surveillance, etc.
  • Information on lab roles, international standards, quality assurance.
  • Guidance on specimen handling, test protocols, assay harmonization.
  • Target audience: Everyone involved in HPV vaccine development/implementation and using HPV data.

2. Recent HPV LabNet Achievements

• Funding: Successful funding secured for RRL/Japan's HPV activities/surveillance.
• Training workshop: Successful HPV genotyping/serology training held in Thailand (Aug 2009).
• Proficiency Studies: 3rd Genotyping Proficiency Study underway. 1st Serology Proficiency Study upcoming.
• Testing: Active implementation of genotype testing confirmation by GRLs and submission by RRLs.
• Collaboration & Data: Pilot study on standardized data reporting initiated. Lab contacts updated in this newsletter.

3. HPV LabNet Training Workshops

• Recent (Thailand, Aug 2009):
  • Objective: Train personnel in SEAR/WPR on HPV testing (genotyping, serology), standardize procedures, prepare for future vaccination.
  • Supporters: GRL/Sweden, RRLs India/Switz, WHO HQ, WPRO/SEARO, UICC.
  • Attendees: Scientists, technicians, health workers (37 local/international participation).
  • Key Topics: Test theory/evaluation, overview/LabManual, QA/QC, surveillance/monitoring.
  • Practice: Used demonstration assays (PGMY-CHUV, VLP-ELISA).
• Upcoming Training:
  • WHO/HQ organizing "2010 WHO HPV LabNet Training Workshop on Genotyping & Serology Performance" in Switzerland (March 2010). Target: 1 participant per WHO region.
  • RRL/Argentina planning a course in PAHO region (April 2010) focusing on regional harmonization and network creation.

4. Typical Activities of an HPV LabNet RRL: Tunisia

• Routine Testing: Introduced HPV testing as routine to all women.
• Genotyping Study: Ongoing collaborative study for genotyping cervical cancer cases (350 samples collected, DNA extracted).
• Awareness: Successful seminars for healthcare professionals, increasing awareness and uptake of HPV testing.
• Research: Investigating HPV interactions/pathways. Increased patients undergoing HPV genotyping.

5. Country Experience - Cervical Cancer Prevention Strategy: Scotland

• Context: Established Scottish HPV Reference Laboratory (SHPVRL) linked to national immunization program launched in 2008.
• Actions:
  • Evaluating HPV prevalence/baseline data in various groups (teenagers, smear attendees, cervical cancer samples).
  • Planning to assess vaccination impact, screen for HPV post-ablation, potentially develop HPV triage/cross-train cytologists.
  • Conducting broad-based HPV testing in other contexts (head & neck cancers).
  • Linking with public health bodies and local implementation groups.
• Outcomes: Achieved high (85%+) vaccine coverage initially, effective communication.

6. Meeting Updates

• Australia Forum (HPV Vaccines, July 2009): Focused on vaccines, testing, screening changes.
• IPV Conference (2009): International gathering covering diverse topics in Papillomavirus research (200+ countries).
• WHO Meeting (Impact Monitoring, Nov 2009, Geneva): Aimed to define objectives (HPV prevalence, lesions, cancer registry data), partners, resources, and clarify endpoints/biologic surveillance for HPV impact assessment.
• IPV Conference (2010, Canada): Upcoming event.
• International HPV-associated Cancers Conferences (Upcoming/2009): Including AOGIN India, EUROGIN Monaco.
