Niger STEPS Survey 2007: Summary
Overview
The Niger STEPS survey on chronic disease risk factors was conducted from December 13-27, 2007, covering all regions except Bilma. It included Step 1 (socio-demographic and behavioral data), Step 2 (physical measurements), and Step 3 (biochemical measurements). The survey targeted adults aged 15-64 years, using a multi-stage cluster sampling design with a sample size of 2,760 participants and a 91.3% response rate. Risk factors assessed included tobacco use, alcohol consumption, fruit and vegetable intake, physical activity, body mass index (BMI), waist circumference, blood pressure, and blood glucose levels.
Key Findings
Step 1: Behavioral and Dietary Factors
- Tobacco Use: Overall, 4.6% of adults smoked tobacco, with higher daily smoking rates among males (3.9%) and females (0.1%).
- Alcohol Consumption: 99.5-99.6% of adults were abstainers, with less than 0.5% reporting current drinking; binge drinking data were insufficient.
- Fruit and Vegetable Consumption: Average intake was low, with fewer than 5 servings per day reported by 96.4-98.0% of adults; males and females showed slight variations.
- Physical Activity: Low activity levels (less than 600 MET-minutes/week) were high at 28.2-31.5%; high activity levels (at least 3000 MET-minutes/week) ranged from 52.5-62.7%. Vigorous activities were rarely engaged in.
Step 2: Physical Measurements
- Body Composition: Average BMI was 21.6 kg/m²; 12.7-16.9% were overweight, and 3.2-5.1% were obese.
- Waist Circumference and Blood Pressure: Average waist circumference was ~80 cm; systolic and diastolic blood pressures were 133.7-136.4 mmHg and 81.3-82.3 mmHg respectively, accounting for medication users. Hypertension prevalence was 36.3% overall, with raised blood pressure classified under ≥140/≥90 mmHg or medication use.
Step 3: Biochemical Measurements
- Blood Glucose: Mean fasting glucose was 5.5 mmol/L; 0.6-1.2% had impaired fasting glycaemia, and 21.8-23.2% had raised blood glucose (≥7.0 mmol/L or equivalent). Other measurements like total cholesterol were inconclusive due to lack of data.
Comprehensive Risk Factors
- Common risk factors included current daily smoking, high BMI, low fruit/vegetable intake, low physical activity, and hypertension (≥140/≥90 mmHg or medication use).
- Percentage with no risk factors: 0.9-1.5%. Percentage with at least three risks: 17.4% overall, peaking at 26.8% in the 45-64-year age group and lowest at 17.5% in 25-44 years.
