1. Global Human Cases

From 2003 to May 7, 2012: 603 laboratory-confirmed human cases, 356 deaths (17 cases in Jan 2012).
Recent (Apr 2-May 7, 2012): 3 new human cases (Cambodia, Egypt, Indonesia).

2. Animal Reservoir

H5N1 circulates endemically in poultry in China, Egypt, Indonesia, Viet Nam, Bangladesh, India, Cambodia.

3. Epidemiology

Cases follow bird outbreak seasons (more in winter, decrease in summer).
All cases linked to exposure to sick/dead poultry, contaminated environments/poultry products.
Risk Assessment: Upward trend expected with poultry outbreaks. No sustained human-to-human transmission detected as of late 2011/early 2012.

4. Specific Cases (Apr 5-Mar 31, 2012 data):

1. A(H3N2)v (Pig Flu - USA)

Recent Case: Utah child, onset Mar 28, 2012; treated, recovered.
Cumulative: 21 cases since July 2009 in USA; 13 post-July 2011 involved 2009 H1N1 genes.
Age: 12 of 21 cases in children under 18.
Immunity & Vaccines: Children under 12 have lower cross-reactive immunity (from mid-90s viruses). Seasonal flu vaccine offers limited cross-protection to A(H3N2)v in adults and none in young children. A specific H3N2v vaccine exists.
Risk: Sporadic cases expected due to virus circulation and flu mutability.

2. A(H10N7)v (Pig Flu - Australia)

Cases: 2 abattoir workers (mild conjunctivitis/symptoms) infected following an A(H10N7) outbreak in poultry.
Outcome: No seroconversion evidence, suggesting limited viral transmission capability or immune response. No onward human-to-human spread reported.
Reportability: Human infections with non-human influenza viruses are reportable under IHR 2005.
