Summary of WHO Risk Assessment on Avian Influenza A(H7N9) Human Infections - 2 October 2014

Key Findings

Number of Cases: As of 2 October 2014, WHO reported 453 laboratory-confirmed cases of human infection with avian influenza A(H7N9), including 175 deaths. Cases were primarily from China (438 cases), with additional cases from Taiwan (4), Hong Kong (10), and one from a Chinese traveler in Malaysia. Since the last update on 27 June 2014, three new cases were reported, including from Hunan Province and Xinjiang Uyghur Autonomous Region.
Virus Characteristics: The virus remains antigenically similar to the candidate vaccine virus derived from A/Anhui/1/2013-like viruses.
Source of Infection: Human infections are associated with exposure to infected live poultry or contaminated environments, such as live poultry markets.
Transmission:
- Human-to-human transmission is difficult and does not support sustained spread; evidence from 14 family clusters (none since the last update) showed low transmissibility.
- The likelihood of additional cases is high, particularly if exposure occurs through poultry contact, and cases may increase during winter or in new regions.
Risk Assessment: The public health risk has not changed since the 27 June 2014 assessment. Further human cases are expected in affected and neighboring areas due to ongoing virus circulation.
International Spread Risk: Travelers from affected areas could introduce the virus abroad, but community-level spread is unlikely as the virus does not transmit easily among humans.
WHO Recommendations: No special entry screening or travel/trade restrictions are recommended. Countries should strengthen influenza surveillance, report all human H7N9 infections, and enhance preparedness.
