American Samoa NCD STEPS Report Summary
1. Introduction
The survey assessed non-communicable disease (NCD) risk factors in American Samoa adults aged 25-64 years using the WHO STEPwise surveillance methodology to monitor trends and guide public health interventions.
2. Key Findings
Behavioral Measures
- Tobacco Use: 29.9% current smokers (38.1% males vs 21.6% females)
- Alcohol Consumption: 63.5% of population consumed alcohol, 49.6% of current drinkers engaged in binge drinking
- Diet: Most individuals met none of the recommended dietary guidelines (mean fruit/vegetable servings=3.8/day), 86.7% consumed <5 servings combined
- Physical Activity: Only 7.8% met physical activity guidelines, 62.2% were classified as physically inactive
Physical & Biochemical Measures
- Obesity: 93.5% were overweight or obese (74.6% obese), mean BMI=34.9 kg/m²
- Blood Pressure: 34.2% had hypertension, higher in males than females
- Blood Glucose: 47.3% had diabetes (3% fasting hyperglycemia, others newly diagnosed)
- Cholesterol: 23.4% had elevated cholesterol (>5.2 mmol/L)
Combined Risk Factors
- 0.4% had no risk factors
- 71.8% had ≥3 major risk factors
- 69.2% of 25-44 year olds had ≥3 risk factors
3. Conclusions
American Samoa faces a severe NCD epidemic with alarmingly high prevalence of major risk factors across multiple domains. Urgent population-level interventions are required to address tobacco use, alcohol consumption, poor diet, physical inactivity and obesity to prevent future disease burden.
4. Recommendations
- Strengthen tobacco control policies through implementation of WHO FCTC
- Reduce alcohol consumption and prevent binge drinking
- Increase fruit and vegetable production/importation
- Enhance physical activity opportunities and healthy eating environments
- Scale up NCD screening, treatment and monitoring
