ICF Checklist Summary
This is the Clinician Form Version 2.1a for the International Classification of Functioning, Disability and Health (ICF). It serves as a practical tool to collect and record information on an individual's functioning and disability. The checklist covers four main domains:
- Body Functions and Structures: Assesses physiological and anatomical impairments, categorized by severity (from 0 to 4 indicating no impairment to complete impairment).

- Environmental Factors: Identifies barriers hindering or facilitators supporting functioning ("Barriers" and "Facilitators"), categorized by severity (0 to 4).
- Other Contextual Information: Includes personal factors and integration of appendix information.
Information can be gathered from written records, primary respondents, other informants, or direct observation. The checklist provides a structured approach for clinicians to assess an individual's health status comprehensively for case records, using severity codes and potentially specifying codes for other categories. Guidance is included for consistent application.
