ICF Checklist Summary

This is the Clinician Form Version 2.1a for the International Classification of Functioning, Disability and Health (ICF). It serves as a practical tool to collect and record information on an individual's functioning and disability. The checklist covers four main domains:

Information can be gathered from written records, primary respondents, other informants, or direct observation. The checklist provides a structured approach for clinicians to assess an individual's health status comprehensively for case records, using severity codes and potentially specifying codes for other categories.
