The U.S. healthcare system showed a recovery in medicine use and spending by 2022, though uncertainty remains due to the COVID-19 pandemic's lingering effects and ongoing healthcare reforms. Health services utilization returned to near 2020 levels, with increased visits and prescriptions but reduced elective procedures and diagnostic tests; telehealth usage declined to 5% of total visits in 2022 from a peak of 26%. Medicine use grew by 2.6%, with biosimilars accounting for 25% of biologic volume, reducing costs and improving patient access. Spending reached $429Bn, driven by specialty medicines like immunology and oncology, which now account for 51% of expenditure. Biosimilars are expected to save over $180Bn by 2027, while policy changes such as the Inflation Reduction Act could slow growth by reducing net prices and capping out-of-pocket costs. Patient out-of-pocket costs increased slightly, with 7% of patients facing high costs, but assistance programs like copay coupons helped reduce abandonment of prescriptions, especially for expensive drugs like insulin. By 2027, spending is projected to stabilize between -2% and 1% growth, with sectors like oncology, neurology, and obesity seeing significant expansion due to new therapies and rising demand.
