Provisional Agenda Item 6: Report on the Evaluation of the Member State Mechanism

1. Background

• The Member State Mechanism on Substandard and Falsified Medical Products was established in 2012 by WHA65.19, aiming to improve collaboration among Member States and WHO to prevent and control substandard/falsified (SF) medical products.
• It is an intergovernmental mechanism reporting every two years to the World Health Assembly.

2. Evaluation Context

• An independent evaluation was requested at WHA76.10 (2023) and conducted by an external team from December 2023–September 2024.
• The evaluation aimed to review progress against objectives since 2017 and provide recommendations for strategic direction.

3. Evaluation Framework

• Used five DAC criteria (relevance, effectiveness, efficiency, coherence, sustainability), plus equity.
• Addressed six main and 13 sub-evaluation questions using mixed methods (desk review, surveys, interviews).

4. Findings

• (1) Relevance: Format is inclusive but struggles to meet operational goals; low participation and funding are barriers.
• (2) Effectiveness: No clear results framework limits impact assessment; Some working groups effective but accountability weak.
• (3) Efficiency: Resource use may be inefficient due to lack of monitoring; Donor dependency and regional imbalance.
• (4) Coherence: Strong alignment with external partners exists but can be improved; Engagement with non-State actors remains cautious.
• (5) Sustainability: At-risk due to heavy donor reliance and unstable funding; Minority Member State contributions limit self-sustainability.
• (6) Equity: Mechanism inherently aims to reduce inequity, yet vulnerable countries' participation remains low.

5. Conclusions

• Greater funding, strategic reforms, and enhanced member participation are needed.
• Collaboration with external stakeholders (including non-State actors) must increase cautiously.
• Sustainability is threatened and requires securing donor commitments and improving performance frameworks.

6. Recommendations

• To Member States: Consider either comprehensive reform (Option A, new structure) or incremental improvements (Option B, enhanced secretariat/expertise).
• To WHO Secretariat: Improve planning, communication, accountability frameworks, and stakeholder engagement; expand technical support and secretariat capacity.
