Care-Seeking Practices
- Self-treatment and home remedies are common, especially among adults. These include herbal concoctions and neem leaf pastes.
- Traditional healers (gunia and baidya) are frequently consulted, often alongside or in place of qualified providers, due to cultural beliefs and convenience.
- Less qualified providers (LQPs) are popular due to their accessibility, use of modern medicine, and flexible payment options. However, they are often untrained and may overprescribe medications for profit.
- Injections are preferred over oral medications because they are perceived as more effective and quicker in delivering relief. Participants associate injections with direct bloodstream delivery and immediate recovery, despite the higher cost and potential side effects of tablets.
- Care-seeking for children and the elderly is more prompt, while for adults, it is often delayed. Infants and children are more likely to be taken to public health centers due to perceived severity and the belief that local providers cannot ensure a full recovery.
Key Determinants of Care-Seeking
The following factors were identified as major determinants of care-seeking behavior:
- Socio-cultural beliefs: Traditional practices and faith in non-qualified providers influence care-seeking decisions.
- Age and gender: Women and children are more likely to seek care promptly, while men tend to delay.
- Trust in service providers: Low trust in community health workers due to drug stock-outs and inconsistent service delivery.
- Proximity and accessibility: Traditional healers and LQPs are more accessible than formal health centers.
- Poverty: Financial constraints affect the ability to purchase nets and medications.
- Perceived effectiveness of services: Belief in the efficacy of treatments and the credibility of providers plays a crucial role.
Policy Implications
- Strengthening community-based organizations can enhance community involvement and ownership in malaria control.
- Improving the availability and reliability of drugs and commodities at the community level can increase trust in the public health system.
- Tailoring health education to address misconceptions and socio-cultural beliefs is essential for effective intervention.
- Promoting active community engagement and addressing the barriers to care-seeking (such as poverty, distrust, and affordability) are necessary for the success of malaria control programs.
Conclusion
While the community has fair awareness of malaria symptoms and prevention, practices remain suboptimal. The study underscores the need for context-sensitive strategies that consider socio-cultural and behavioral factors, and enhance service accessibility and trust to improve community response to malaria control interventions.
